The Early Intervetion Data Logging project was sponsored by the Infant Evaluation Program. The Infant Evaluation Program provides therapy services to children from birth until age 3. The children can receive physical, occupational, speech, or other kinds of therapy depending on what their individual needs are. The problem that we were tasked with correcting lies in the Program's methods of record keeping. Currently, all records are kept as hard copies in patients' individual files, making looking up information difficult. To combat this problem, we were tasked with creating a database of pertinent information about both patients and therapists. Then, a database user can generate a report of basic information about a patient (referred to as a face sheet) that includes demographic information, sources of funding, types and frequency of therapy received, and the dates of upcoming semi-annual reports of progress. The same type of report can be generated for an individual therapist. In the therapist's case, it shows all the patients a therapist currently sees, with what frequency, and when each patient's next report is due.
